Sadly we do not have this option of a per-document visibility control. The document and attachments visibility will be the same of the item itself. We have discussed this previously, there is even an issue for that:

By now, the only solution is really to keep the whole item private. Another strategy that I can think of is to keep your media in a collection and bind them to the item via relationship metadata… then you can have the relationship private and the item public. But its not ideal, I admit.
